In collaboration with physicians, nurse practitioners, and other clinical and office staff members, the Triage RN is responsible for the provision of direct and indirect nursing care including physical, psychological and social assessment which requires nursing judgment, intervention, referral or follow-up, health maintenance, teaching, counseling, planning and restoration for optimal functioning and comfort of the patients we serve. This position requires a high degree of flexibility, independence, critical thinking and willingness to collaborate with patients and families to enhance the delivery of patient care services across the continuum of care.

The triage RN will work in a variety of settings of the Cardiovascular Heath Practice. As the majority of this role is telephone and internet/EMR based, the triage RN will work with all patients and clinicians of the entire practice regardless of the location in which the providers see the patients. The primary workspace for the triage RN will be located in the Needham, MA office. Other practice locations in which the triage RN will work including Chestnut Hill, MA and Wellesley, MA. At all of these locations, there will be some patient facing encounters as detailed below. On occasion, at the discretion of the Medical Director and Practice Manager, hybrid telemedicine work from home may be required. Other than during the initial training period, the triage RN will not be working at the Newton-Wellesley Hospital campus. As that is the largest practice location, the triage RN will be working with those patients and providers regularly, but from an off-campus location.

ESSENTIAL FUNCTIONS

  • In collaboration with physicians and clinician colleagues, participates in the provision of direct and indirect patient care utilizing the nursing process including nursing assessment, problem identification and intervention.
  • Receives patient problem telephone calls and electronic clinical portal messages and performs nursing assessment, identifies patient problem, collaborates with clinician (physician and/or nurse practitioner) to plan intervention, communicates instructions and education to patient and documents in electronic medical record. Ensures resolution of issue with consistent and clear follow up.
  • Assess and triage EKG/Arrhythmia monitor tracings from patient devices/ambulatory monitoring companies then collaborates with clinical team including cardiologists/NPs/PAs to plan intervention, communicate instructions and education to patient and to document plan in electronic medical record. Ensures resolution of issue with consistent and clear follow up.
  • Completes prior authorization insurance company requirements.
  • Completes prescription refill review and electronic ordering processing.
  • Coordinates with support staff to schedule same day urgent patient appointments within the practice, emergency department, urgent care center or physician specialist office.
  • Coordinates with support staff, physicians, nursing staff, scheduling to schedule appointments, testing or procedures.
  • Provides patient with diagnostic test results by telephone and/or electronic secure messaging portal and documents in electronic medical record,in collaboration with physician and/or nurse practitioner,
  • Provides patient with peri-procedure instructions (ie; medication, wound care), in collaboration with physician and/or nurse practitioner,
  • Coordinates anticoagulation therapy management with physicians and/or nurse practitioner, communicates dosage and laboratory testing schedule to patient and documents in electronic medical record.
  • Performs in-person patient care visits including: subcutaneous injection education (ie; PCSK-9 inhibitors and Enoxaparin), blood pressure cuff calibration appointments, and any point-of-service testing wherever applicable and documents in electronic medical record.
  • Coordinates patient home care services and required paperwork including VNA, IV Therapy, Elder Services and DME.
  • Obtains and communicates critical laboratory and radiology results to physician and/or nurse practitioner, communicates to patient with instructions, as needed and documents in electronic medical record.
  • Coordinates post-hospital, post-procedure, ER, SNF and other outside facility appointment scheduling and documents in electronic medical record.
  • Completes forms( such as FMLA, Disability, Prior Authorization) using chart notes for physician signature.
  • Coordinates and communicates patient transfers to emergency department and other ambulatory or hospital based clinics.
  • Communicates and coordinates high-risk patient care with the iCMP team, as needed.
  • Reviews and documents advanced directives in electronic medical record.
  • Assess unscheduled patients arriving with an illness or medical problem following practice triage protocols.
  • On occasion, rooms patients and performs vital signs and 12 lead EKG.
  • Reviews and contacts patients who cancelled same day illness visit and document

  • patient status in electronic medical record, as needed.
  • Participates in all relevant hospital,quality and practice programs and protocols as assigned.
  • Administers physician ordered medications as required by practice policy.
  • Complies with all aspects of HIPAA and Information Security Plan regulations.
  • Complies with all local, state and federal guidelines that govern nurse practice and licensure.
  • Demonstrates excellence in quality patient care and quality service.
  • Demonstrates effective coordination and communication skills between clinical and non-clinical staff.
  • Acts as a resource person for staff and seeks guidance from supervisor, as needed.
  • Plans personal professional development.
  • Maintains neat and orderly workstation.
  • Performs other duties, as assigned.


  • Qualifications
    Strong interpersonal communication, problem solving and conflict management skills required

    Graduate of an accredited school of nursing required

    Current Licensure as a Registered Nurse in MA required

    Bachelor Degree in Nursing preferred

    3-5 years of FOCUSED cardiac nursing experience Required

    Demonstrated clinical competence in EKG interpretation, non invasive cardiology study (ie; stress testing, echo, nuclear, pacemaker, holter and mobile cardiac telemetry reports) Cardiac pharmacology, and oral anticoagulation management.

    Evidence of continued education and professional development

    Certification or advanced training in specialty area preferred

    Telemetry training required
