Benchmark Human Services is a national leader in the disabilities and behavioral health field and is looking to expand its support coordination (case management) operations into Utah. The organization has operated for over 60 years and its reputation for providing quality has enabled the company to grow and offer a diverse range of services. We are seeking Support Coordinators throughout the state of Utah.

Job Responsibilities include but are not limited to:

  • Follow all policies and procedures set forth by the company and Utah Department of Human Services, Division of Services for People with Disabilities and Home and Community Based Services (HCBS) rules
  • Establish and maintain the participants eligibility in the support system and Waiver
  • Coordinate the delivery of quality services
  • Maintain Medicaid financial and categorical eligibility
  • Identify the supports necessary to insure the participant's health and safety
  • Develop and assure the implementation of the Person Centered Support Plan (PCSP)
  • Provide initial assessment and ongoing assessments for level of care
  • Provide ongoing monitoring of PCSP
  • Provide any transitional and discharge planning
  • Assist individual to locate safe and affordable housing
  • Attend required meetings
  • Complete required documentation
  • Establish and maintain positive relationships with individual, families, guardians, state officials and team members
  • Comply with quality outcomes for consumers and the department
  • Coordinate meetings with individual, families and guardians, and members of the interdisciplinary team as required
  • Record work and billing time in accordance with company policies
Qualifications:
  • Bachelor degree in human services or a related field
  • A minimum of 1 year working in a support coordination role within the Utah system
  • Preferred credentials include Accredited Case Manager (ACM) or Certified Case Manager (CCM)
  • Effective written and oral communication skills
  • Successfully complete the state's annual training
  • Reliable transportation, valid Utah driver's license, and auto insurance
