Description:
Provides coordination and lateral integration for the Stroke patient between clinicians, management and patient care activities consistent with our MAGIS patient/family Centered Model of Care to achieve high standards of patient care and quality outcomes. Works with the multidisciplinary team including physicians, staff, internal and external partners to insure patient's progress along the continuum of care in an efficient and cost effective manner that ensures quality outcomes. Utilizes established evidence based guidelines to support appropriate level of care throughout the hospitalization and beyond. Acts as a patient advocate and communication link with other departments, and community resources. Facilitates multidisciplinary rounds and supports nursing staff in development of individualized, evidence-based outcome oriented, safe and timely practices. Assures compliance with regulatory agencies, i.e., The Joint Commission, AHA (Get with the Guidelines) applies 14 forces of Magnetism and supports the Magnet Model. Collaborates with Loyola Medicine partners to integrate and optimize delivery of stroke care across the region.
Position Requirements:

Minimum Education:
Required: Bachelors Degree

Preferred: Masters Degree

Specify Degree(s): Nursing

Minimum Experience:
Required: 3-5 years of previous job-related experience
Preferred: 6-10 years of previous job-related experience

Licensure/Certifications:
Required: CPR (Cardiopulmonary Resuscitation) & Current Registered Nurse License State of Illinois
Preferred: Specialty certification in clinical or functional area of nursing granted by a national nursing organization.

Position Responsibilities:

Works with the multidisciplinary and regional teams including physicians, staff, internal and external partners to insure patient's progress along the continuum of care in an efficient and cost effective manner that ensures quality outcomes. Utilizes established evidence based guidelines to support appropriate level of care throughout the hospitalization and post-hospital care. Participate in facility clinical research trials as designated by the program medical director. Support monthly stroke support groups.

Facilitates multidisciplinary rounds and supports nursing staff in development of individualized, evidence-based outcome oriented, safe and timely practices.

Assures compliance with regulatory agencies, i.e., The Joint Commission, AHA (Get with the Guidelines)Serves as coordinator for stroke Data Base.

Works with the multidisciplinary and regional STROKE Teams including physicians, staff, and administration members of the CCE, to insure patient's progress along the continuum of care in an efficient and cost effective manner that ensures quality outcomes. Utilizes established evidence based guidelines to support appropriate level of care throughout the hospitalization and beyond. Is an active participant in all Stroke related QI activities.

In partnership with the Stroke Medical Director and hospital administration, carry out the administrative duties required for LUMC's stroke program to function properly - curriculum development, team management and process improvement. Acts as the program liaison with external hospital partners through our Telestroke relationships. Collaborates with stroke teams throughout Loyola Medicine.

Maintain all aspects of the stroke program, including concurrent review of medical records, concurrent PI, registry input, stroke prevention, community liaison, committee participation and other activities which enhance optimal stroke care management.
