Description:
Provides the following to all age groups of patients treated at Holy Cross who are referred or request services:
• Complex transition management
• Psychosocial assessments on patients with identified needs.
• Supportive counseling services for patients and families.
• Referral of patients to community agencies/resources.
• Support/assistance to hospital staff regarding the reporting of child abuse/neglect or the abuse/neglect/exploitation of vulnerable adults

Description:
  • Reporting to the Manager of Social Work
  • Social Worker will provide assessment, counseling, referral and discharge planning services to all age groups of patients treated at Holy Cross; provides supportive services to patients and their families.

Responsibilities:
  • Social Workers are focused on discharge and initial inpatient admission and appropriate community-based services.
  • Reviewed clinical assessments and determining appropriate levels of care, determining if additional clinical services are needed for infants to elderly patient population.
  • Effectively handled member crisis calls by identifying risk, ensuring safety, and coordinating timely access to care.

What you will need:
  • State of Maryland LSWA, LGSW, LCSW or LCSW-C licensure (or license eligible).
  • Bachelors or Masters Degree in Social Work.
  • One year of social work experience in an acute-care setting preferred. Demonstrated ability to work with all age groups of patients, with an emphasis on adult and geriatric patients. Able to communicate effectively.
