The Jewish Board's Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities. Crossroads offers a space of hope and empowerment by providing mental health support in a safe, inclusive environment. Services are provided along a continuum of settings including a young adult outpatient clinic for individuals 14-26, an OnTrackNY program for individuals 16-30 experiencing first episode psychosis, 8 school-based satellite programs for young people from K-12th grade, and a satellite at The Jewish Community Center.

The social worker position is responsible for providing developmentally appropriate, evidence based interventions to clients and families; crisis intervention; advocating for clients/families; and coordinating with other providers engaged in the client's care to maximize treatment outcomes. The social worker is responsible for providing treatment as part of a therapeutic team in the young adult Article 31 mental health clinic setting.

Responsibilities include but, are not limited to:

  • Provide evidence-based treatment interventions, inclusive of their families and support systems. Modalities include individual, groups, family, and collateral treatment for young people age 14-26.
  • Crisis intervention when indicated as part of the client's treatment.
  • Outreach to clients/families to establish care as well as increase or maintain engagement in care
  • Care coordination with outside treatment providers/systems with the goal of improving client outcomes
  • Maintain accurate and up-to-date case records, e.g. progress notes, assessments and treatment plans, statistics, and other documentation as required by The Jewish Board and other governmental licensing and funding entities.
  • Maintain LOV requirements and manage caseload around case closures and transitions.
  • Active participate in supervision, training and case consultation.
  • Attend scheduled staff meetings and clinical team meetings.
  • Engage in continuing education activities
  • Other duties as assigned

Qualifications

The Jewish Board is currently looking for talented professionals of all cultures, religions, races, and gender expressions with the following qualifications:

  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
  • Available 2 or more weekday evenings until clinic close.
  • A minimum 1 year of relevant experience with similar population/setting.
  • Fluency in Spanish preferred.
