ABOUT THE JOB:

The Healthy Aging Program utilizes a focused and strategic approach to provide services for adult residents ages 60+ living in Jewish board supportive housing programs to help them age well and safely in their home for as long as possible. Our goal is to incorporate an aging lens into our supportive housing programs, infusing senior services into our work enabling older residents to continue living in their homes as they age through staff training, education, community-based and in-home services partnership development. Staff use an approach based on the "Stages of Change" with our residents to work with them at their pace and comfort level towards creating the specific changes they need to help them age in place.

The three key program elements to meet our residents' needs through this Healthy Aging program model are:

1. Coordination, linkage and support services for aging residents

2. Training of supportive housing staff in geriatric care, services, and benefits and related topics

3. Community-based care collaborations and in-home care provider relationships

The Social worker is responsible for superior practice and/or the assumption of additional or special responsibilities. This position is available only through promotion from within the staff. Perform casework for clients. Evaluate case data. Perform treatment processes. Analyze and evaluate course results of treatment processes.

KEY ESSENTIAL FUNCTIONS:

  • Obtain, analyze and evaluate case data.
  • Carry out treatment processes.
  • Analyze and evaluate course and results of treatment processes.
  • Record material and case summaries.
  • Utilize Agency resources as well as community resources to perform related tasks.

TO QUALIFY

The Jewish Board is currently looking for talented professionals of all cultures, religions, races, and gender expressions with the following qualifications:

You should have:

  • Master's Degree in Social Work (MSW) and LMSW/ LCSW certification.
  • Two years of experience in a relevant field. Prior experience with supportive housing population preferred.
  • Prior experience in geriatric services and supportive housing population preferred.
