Job Summary:

One (1) Full-Time position
As a Service Coordinator, you will helps answer families concerns about their infants and toddlers' development by facilitating developmental evaluations. The Service Coordinator works in partnership with developmental specialists including speech therapists, physical therapists, occupational therapists, social workers, and developmental therapists. You will help families link to the supports their children need in order to develop to their greatest potential. You will work with children until they reach three years old and then help the family transition to a preschool or special education program.

  • Completing Intakes, Coordinating eligibility determination process
  • Assisting families in locating service providers,
  • Facilitating development of individual family service plans (IFSP)
  • Maintaining early intervention records
  • Maintaining up to date accurate information on Cornerstone system,
  • Maintaining on-going contact with families, Facilitating six month IFSP reviews
  • Implementing transition plan
  • Completing the family fee schedule
  • Conducting developmental screenings and public awareness presentations
Job Requirements:

What we need from you:
  • Bachelor's degree in Social Work, Psychology or related field required.
  • Minimum of 1 - 3 years working either in Early Intervention or with families of children with special needs required.
  • Must become credentialed as a Service Coordinator.
  • Employee must maintain automobile insurance.
