About the job:

The primary purpose of the Nurse Manager is to supervise the day-to-day activities of the assigned nursing station(s). The Nurse Manger is responsible for the coordination, planning implementing, and evaluating the nursing services provided to each resident on a 24-hour basis. This role also supervises the direct care staff assigned to the Nurse Manager's Unit/Household.

Essentials:

Nursing Care Functions:
  • Directs and oversees the nursing staff to ensure the nursing procedures are followed and reflect the current policies and procedures of the facility.
  • Follows VOANS core clinical systems.
  • Monitor medication passes and treatment schedules to assure that medications and treatments are being administered as ordered.
  • Perform direct care related activities as the need arises.
  • Participates in the development of policies and procedures pertaining to resident care.
  • Perform daily rounds of the nursing unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Perform QAPI rounds.
  • Assist in developing methods for coordinating nursing services with other resident services to ensure that continuity of the resident's total regimen of care.
  • Participate in the development, maintenance and implementation of the facilities QAA/QAPI process.
  • Complete and investigate accident/incident reports and assist with abuse/neglect reporting to specific agencies as needed.
  • Acts as a liaison between physician or other provider, resident representative, family and resident care team to ensure physician orders are accurately carried out and documented using the information from the resident's plan of care.
  • Ability to create the CMS 802 Roster Matrix in preparation for the weekly interdisciplinary meetings.
  • Initiate and resolve follow-up and problem charting as needed.
  • Participates in weekly resident and family care conferences.
  • Brings concerns/improvement suggestions to the facility leadership QAA/QAPI committee.
Care Plan and Assessment Function:
  • Review new admission and transfers for appropriate assessment and documentation.
  • Assure completion of all necessary assessments required for the RAI process.
  • Ensures the development/completion of person-centered care plans for assigned residents err state/federal guidelines.
  • Participation in the development and ongoing review of the plan of care for each resident for appropriate resident goals, problems, approaches and revisions based on nursing needs.
  • Assist in the monitoring and assessment of seriously ill residents as needed. Utilizes the E-Interact COC assessment and care paths.
  • Determine and authorize with the interdisciplinary team the appropriate use of opioids, chemical and physical restraints/devices when indicated in accordance with facility policy and state/federal requirements.
  • Review nurses' notes, NA charting to ensure compliance with the resident plan of care.
  • Complete and/or participate in the MDS/RAI process and/or clinical reimbursement as determined by facility policy. Review for accuracy and timeliness per State/Federal Requirements.
  • Direct communication with the MDS/PPS Coordinator and/or Reimbursement Nurse or Specialist.
Committee Functions:
  • Serve on, participate in and attend various committees and meetings of the facility to which you are assigned.
Workforce Functions:
  • Assists with the coordination of staffing patterns and productive nursing hours as delegated by the Director of Nursing.
  • Responsible for holding staff accountable for correctly following policies and procedures as outlined in the employee handbook and nursing job descriptions.
  • Completes employee evaluations in a timely manner as directed.
  • Assists with disciplinary actions as assigned by the Director of Nursing.
  • Coordinates and conducts routine nursing staff meetings on assigned unit.
  • On-call as delegated by the Director of Nursing Services.
  • Attends required in-services and completes assigned on-line modules.
Staff Development:
  • Assist the Staff Development Coordinator or Director in identifying ongoing educational and training needs.
Environment, Safety and Sanitation:
  • Follow up on all event reports and give to Director of Nursing within facility time protocol.
  • Observes staff for proper safety techniques and Antibiotic Stewardship, Infection Prevention and Infect Control policies and procedures.
  • Ensure the environment on the nursing unit is clean, safe, clutter-free and homelike.
  • Supervision of meal service to ensure quality of life with regards to the dining experience.
  • Ensure IPC procedures are followed by staff.
Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Current licensure in the state of employment as a Registered Nurse. Current CPR Certification.
  • Registered Nurse with previous long-term care experience.
  • Good problem solver with abilities to make independent judgments utilizing initiative, discretion and the nursing process as needed.
  • Demonstrates leadership ability.
  • Effective communicator with staff, families, Physicians/Providers, Surveyors and outside agencies.
  • Ability to prioritize duties.
  • Customer service oriented with knowledge of successful customer service techniques.
  • Ability to complete assigned duties within a constantly changing environment.
  • Must possess effective management and supervisory skills.
  • Clinical competence to utilize the nursing process to perform physical and cognitive assessments of residents.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  • Able to handle stressful situations. Able and willing to work flexible hours.
  • Understanding of Antibiotic Stewardship, Infection Prevention, Infection Control and OSHA guidelines.
  • Knowledge of state and federal regulatory requirements for long term care.
