Description:
The Living Donor Transplant Coordinator supports the attending physician in education of the kidney or liver donation process for interested prospective donors and the coordination of care according to pre-determined clinical protocols for kidney or liver donors pre and post transplant. This position may
be required to be on-call \"24/7\"for planned kidney transplants as part of the National Kidney Registry (NKR) or swaps involving other organ exchange networks.

Position Requirements:

Minimum Education:

Required: Bachelors Degree

Specify Degree(s): RN or BSN.

Minimum Experience:

Required: 1-2 years of previous job-related experience

Preferred: 3-5 years of previous job-related experience

Details: Knowledge of solid organ transplantation, living donor process or staff nurse from critical care or med/surg units that care for transplant recipients. Licensure/Certifications:

Required: CPR (Cardiopulmonary Resuscitation) Current Registered Nurse License State of Illinois

COORDINATION OF CARE - Assists in the coordination of care for the potential kidney or liver donors based on pre-determined clinical protocols as part of the pre-donation evaluation and post-donation for kidney or liver transplant. Collection and review of diagnostic tests, laboratory work and other pertinent information with the transplant team physicians. Maintains the appropriate required documentation of the physician orders and communications pertaining to procedures and patient medical management in the progress note, flow sheet entry and transplant database. Maintains the Electronic Health Record patient and donor roster(s). Collaboration of the multi-disciplinary team including social work, physicians, financial coordinators, nursing staff, administrators, etc. Assists in presenting pre-transplant work-up to the living donor selection medical review board (MRB. Triage calls and works collaboratively with the transplant team physicians in health care management needs. Attendance and participation in assigned committee meetings (e.g. Medical Review Committee). Submits clinical information as requested to insurance carriers/payers for precertification/authorization of care. Provides support and cross-coverage to RN transplant coordinator team in their specified clinical areas as needed.

EDUCATION - Development and maintenance of teaching materials as well as education/teaching of living donor education to patients, patient families, staff nurses and other departmental staff, external clinical providers. Educates prospective donors on all aspects of the living donation transplant process and documents in the living donor's medical record and discussions that include but are not limited to the following: Consent process, Evaluation process, Surgical procedure. Benefit and need for long-term medical follow up. Grievances/complaints. Elevate questions/concerns to the chain of command when the Living Donor Transplant Coordinator cannot answer questions, concerns or complaints.

DATA PREPARATION AND SUBMISSION - Assists in collecting, reporting and submission of transplant related statistics to United Network for Organ Sharing (UNOS), Centers for Medicare and Medicaid Services (CMS), The Joint Commission (TJC), National Kidney Registry (NKR), and the Transplant Information Electronic Data Interchange (TIEDI) and Organ Procurement and Transplantation Network (OPTN) data entry system for transplant programs and any other relevant agencies.

QUALITY/ADMINISTRATIVE - Performs administrative functions such as preparation of data for quality review, attending protocol meetings, transplant conferences, and other administrative duties upon request. Actively participates in the development and operations of the Solid Organ Transplant Program and in the Quality Assurance/Performance Improvement meetings. Participate in the orientation of new hires to the transplant coordinator team.

OUTREACH - Actively participates in transplant program community outreach and able to travel to those facilities as it pertains to the living donation process.
