Responsibilities

Position Overview:

The High Risk RN Coordinator serves as an advocate and assists patients to reach optimal function and health in an appropriate environment that supports their needs. This position provides coordination across the continuum of care, from inpatient to outpatient to community. The High Risk RN Coordinator applies the process of assessment, care planning, implementation, monitoring, evaluation and coordination of care to meet the patient's health care needs across the continuum in collaboration with Primary Care Physicians, interdisciplinary care team members and social service organizations to ensure safe, quality and cost effective care.

Qualifications

Required:

  • Current CA Registered Nurse License
  • Five (5) years previous hospital and inpatient/outpatient medical case management experience
  • One (1) year direct patient care nursing experience
  • One (1) year of community/population health or related experience
  • Current Driving License
  • Current and ongoing Motor Vehicle Report (MVR) that is considered insurable under the CMHS insurance coverages
  • Strong analytical/problem-solving, written and verbal communication skills
  • Proficient in Microsoft Office Applications
Preferred:
  • Bachelor's or Master's Degree in a related field
  • Bilingual in English/Spanish
  • Practice experience in evaluation and management of complex medical and psychosocial issues
  • Demonstrated skills in clinical quality improvement and in resource management
  • Previous Project Management and Quality Improvement experience
  • Previous Electronic Health Application (EHR) experience (Allscripts & TouchWorks)
