LIFE St. Joseph of the Pines is currently seeking a RN Case Manager. Under the supervision of the Nurse Manager, the RN Case Manager is responsible for planning, coordination, implementation and evaluation of health care and social services to participants at the PACE program. Serves as the liaison between the interdisciplinary team (IDT), participants, caregivers and community agencies.
LIFE St. Joseph of the Pines, located in Fayetteville, North Carolina is a Program of All-inclusive Care for the Elderly (PACE). PACE is a unique program, which can assist elders to live safely at home or with their loved ones with the help of a team of health care professionals. PACE provides a full spectrum of health care services for frail elderly individuals certified to require nursing home care, and it is centered around the belief that it is better for seniors with chronic care needs and their families to be served in the community wherever possible.

LIFE St. Joseph of the Pines is currently seeking a RN Case Manager. Under the supervision of the Nurse Manager, the RN Case Manager is responsible for planning, coordination, implementation and evaluation of health care and social services to participants at the PACE program. Serves as the liaison between the interdisciplinary team (IDT), participants, caregivers and community agencies.

Duties and responsibilities include, but are not limited to:

  • Working knowledge of the PACE regulations
  • Broad knowledge base of health care problems of the frail and elderly
  • Acts as liaison between the interdisciplinary team (IDT), participants, caregivers and community agencies.
  • Coordinate hospital discharges in conjunction with the interdisciplinary team (IDT) and attending physician. Communicates updates to family or caregivers as needed.
  • Perform home visits as needed to assess living environment and support system.
  • Maintain current written/electronic records and documents related to participant care.
  • Communicate, counsels and guides participants and families regarding service needs and concerns; makes referrals as appropriate.
  • Manage and facilitate coordination and collaboration of participant care with all clinicians and caregivers. Responsible for identification of need and utilization of clinical services.
  • Actively participates in the coordination of efforts towards survey preparedness. When necessary contribute to the plan of correction for department deficiencies.

Education and experience:
  • Current North Carolina Registered Nurse (RN) License
  • CPR certification is required.
  • Minimum of two (2) years of demonstrated successful experience in related services with geriatric population.
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
