GENERAL SUMMARY/ OVERVIEW STATEMENT:

Under the supervision of the Department of Orthopaedic Surgery Chief, Director of Orthopaedic Advanced Practitioners (APPs), and in accordance with Massachusetts state regulations

governing Physician Assistant/Associate (PA) practice, the American Academy of Physician Assistants/Associates and the National Commission on Certification of PAs, the PA will be responsible for

weekend coverage of all Orthopaedic primary patients in the inpatient setting. The role is like a hospitalist position whereby the PA is not assigned to one service, rather covers all: Oncology, Trauma, Arthroplasty, Hand/Upper Extremity, Foot & Ankle, Shoulder, Spine, and Sports. Weekend coverage is shared with one resident.

The PA is designated as Responding Clinician and is responsible for the care of the primary Orthopaedic inpatient population (approx. 50-70 patients). This includes monitoring postop care, respond to patient emergencies, and assist with discharges. The PA, when necessary, will take patient histories, perform physical examinations, arrange for diagnostic procedures, diagnosis and treat various conditions, prescribe medications, order imaging and all types of testing procedures, coordinate consultations with multiple disciplines, and most important, monitor postop care in accordance with orthopaedic surgeons' plans. Discharge planning is an essential responsibility which requires communication with case management, PCP/referring physicians and all involved in the transition of care (consults, skilled facility, VNA). The PA has a significant role relative to patient/family education.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Acts as responding clinician for the entire inpatient orthopaedic census, shared with one resident.
  • Conducts verbal sign-out with night intern(IPASS), and documents via orthopass.
  • Provides direct care, counseling, and teaching to the inpatient orthopaedic primary patients.
  • Performs complete histories and physical examinations.
  • Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
  • Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
  • Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
  • Responds to emergent patient issues (chest pain, SOB, wound dehiscence, syncope, falls, social-behavioral issues, codes)
  • Consistently provides high quality and timely documentation including admission and progress notes, procedure notes if necessary, and discharge summaries.
  • Performs bedside procedures as are appropriate to the patient population.
  • Practices cost effective medicine in an efficient manner, maximizing available resources, and expedites discharges in a timely manner.
  • Discusses patient data with other physicians and professionals as appropriate in a multidisciplinary setting.
  • Demonstrates professional collaborative and consultative relationships with inpatient Orthopaedic APP colleagues, residents, Fellows, and other multidisciplinary providers.
  • Follows all designated prescriptive practices and monitoring requirements with the designated supervising physician and administrative report.
  • Participates in departmental and organizational committees and programs as appropriate and attends all required meetings, in-services, and professional trainings.
  • Maintains superior interpersonal and communication skills as a member of the healthcare team to collaborate effectively with patients, families, staff, and community health workers.
  • Remains actively involved in continuing education with a commitment to self and departmental growth.
  • Adheres to all established safety policies, procedures, and precautions; identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation.
  • Demonstrates understanding of procedures, policies, and documentation required to ensure compliance with hospital standards of care.

  • Must have the ability to work in a high stress, dynamic, acute clinical setting with changing organizational climate.
  • Demonstrates expert clinical judgment skills to function in an independent role.
  • Must have the ability to be self-directive and to demonstrate initiative in addressing the responsibilities of the position.
  • Must have the ability to utilize appropriate problem-solving and conflict resolution skills.
  • Must excel in effective and positive communications with staff nurses, case managers and many interdisciplinary providers.
  • Must be energetic and quality driven and have a proven ability to be a productive member of a team dedicated to providing optimal patient care.
  • Must be proficient with MGH computer systems as needed to independently perform daily tasks (Epic preferred).

  • Must maintain the highest degree of professional judgment under the collaborating supervising physician and Director of Orthopaedic APPs.

WORKING CONDITIONS:

The Responding Clinician role covering the entire primary Orthopaedic inpatient census requires excellent interpersonal skills to work with residents, nurses, and various disciplines (physical & occupational therapy, case management, medicine, and specialty consults). The environment is fast-paced and involves walking to and from patient care units, clinical and non-clinical activities, and computer work (patient care orders, discharge summaries). As in any health care facility, the work involves possible exposure to infection, instruments, monitoring devices, and other state-of-the-art equipment.

Qualifications
  • Completed a graduate level physician assistant program (Master's degree) accredited by the Accreditation and Review Committee for Physician Assistants (ARC-PA) or such other accrediting agency that qualifies a graduate to sit for the NCCPA certifying examination.
  • Current certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • Current valid licensure as a physician assistant in the Commonwealth of Massachusetts.
  • Must complete 100 hours of CME according to national and state regulations during each two-year period of employment.
  • In addition, PA must demonstrate continued competence by passing a re-certification exam every ten years.
  • Current DEA registration and Massachusetts Controlled Substance Registration (MCSR).
  • Qualified in all basic and advanced cardiac life support (ACLS) standards as established by the American Heart Association.
  • Previous experience as a Hospitalist or at least 5 years' plus experience in the Advanced Practice role, inpatient setting.
