Provides pharmaceutical care to patients at home, in accordance with ASHP guidelines and Infusion Pharmacy policies and procedures. Assists in the development and implementation of a safe, functional and patient-orientated clinical pharmacy and infusion service under the guidance of Infusion Pharmacy Manager and Director of Infusion Services. When assigned, be responsible for the coordination of all infusion production pharmacy operation per policies and procedures and USP 797 guidelines. Oversees technicians, coordinates delivery schedules with customer service representatives and warehouse personnel. Reports to Infusion Pharmacy Manager.

Education

• B.S. in pharmacy; Pharm D. Preferred
• Licensed to practice in Maryland

Required Licensure, Certification, etc.:

• Maryland Pharmacist License

Work Experience:

• Minimum of 6 months of infusion experience within the last 3 years.
• Thorough knowledge of the uses of IV medications, especially their application in the home are setting
• Experience in patient care and patient medication counseling
