Position Overview: The Older Adult Behavioral Health (OABH) Clinical Specialist will engage community partners in the fields of geriatrics, disability mental health, and addiction services in promoting and creating positive collaboration towards the end of reducing gaps in service coverage to community members 60 years and over who are experiencing behavioral health issues.

Essential Responsibilities:

  • Conduct needs assessment of the three county region (Crook, Deschutes, and Jefferson) to identify and actively implement plans to eliminate system barriers for older adults and people with disabilities as they successfully integrate into their own communities.
  • Provide coordination and collaboration between systems in order to promote healthy living.
  • Respond to individuals with urgent/acute needs by utilizing the appropriate resources to assist case managers and mental health staff in managing the need.
  • Assist with transitions and discharges from long term care, may include diversions and /or discharge from jails and hospitals; coordinate ongoing development for joint service delivery between Aging and People with Disabilities and behavioral health providers;
  • Develop and maintain ongoing working relationships between local, regional and state agencies as well as advocates, individuals, and their family members in order to provide the full range of appropriate services for each individual.
  • Engage with various interdisciplinary teams, individuals, and families to develop best practices models for older adults and people with disabilities who live in the community. Ensure these services are available and are provided with dignity and responsiveness.
  • Provide information as needed to families, primary care providers, and local communities regarding local, regional, and state wide services for older adults with behavioral health needs.
  • Create reporting elements specific to the coordination of services. Document data based on identified outcome elements.
  • Train and inform local community partners in the use of specific evidence based practices.

Supporting Responsibilities:
  • Meet department and company performance and attendance expectations.
  • Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.

Work Experience: A minimum of 5 years' experience in mental health or aging/disabilities with emphasis on Geriatric/older adult care required.

Education, Certificates, Licenses:Master's Degree/Qualified Mental Health Professional (QMHP) required, with an emphasis in social work, psychology, sociology, or a closely allied field. Licensed Clinical Social Worker or other Oregon licensure highly desired.

Knowledge: Excellent communication skills, both verbal and written. Flexible; able to multi-task in response to time-sensitive and changing situations. Effective listening skills. Knowledgeable of best/promising practices in the field of older adult services, specifically those that address behavioral, developmental, and physical health service needs. Objective, solution-oriented problem solving skills. Able to handle highly sensitive and confidential information. Able to network and connect people to appropriate resources. Comfortable using MS software products; PC (or Mac) experience required. Strong planning and organizational skills. Effective presentation skills, including public speaking and meeting facilitation. Stays positive and solution-oriented. Discovers underlying issues in situations and re-states them in a helpful way.

Environment: Work inside in a general office setting with ergonomically configured equipment. Travel is required approximately 10 % of the time.

Physical Requirements: Stoop and bend. Sit and/or stand for extended periods of time while performing core job functions. Repetitive motions to include typing, sorting and filing. Light lifting and carrying of files and business materials. Ability to read and comprehend both written and spoken English. Communicate clearly and effectively.
