Position: Diabetes Program Manager

Schedule: Monday - Friday, 8:00am-4:00pm

Job Summary:

The Nurse Manager of Diabetes Program functions as the first-line administrative nursing leader, responsible for the financial and operational management of the Diabetes program and educator staff. The Nurse Manager of Diabetes Program directly supervises the staff to include the Diabetes Educators and Administrative support staff. She/he is responsible for overseeing the day to day operations of the Diabetes Education Department, both inpatient and outpatient. The Nurse Manager of Diabetes Program provides oversight of the Diabetes Education Staff, monitoring and facilitating maintenance of staff qualifications and providing consultative review and recommendations on diabetes clinical management to the Diabetes Education staff to ensure quality and individualized patient care. The Nurse Manager of Diabetes Program oversees the planning, implementation, and evaluation of the Diabetes Self-Management Education (DSME) accredited program at all sites and ensures that the National Standards for DSME are met and maintained at all times. The Nurse Manager of Diabetes Program is responsible for the development and evaluation of diabetes patient education across the continuum of care to ensure quality, patient-centered education in the ambulatory/primary care, community, and inpatient settings. Ensures that educational materials are reviewed for health literacy and cultural; competence. The work requires knowledge of the policies, procedures, and regulations that govern nursing practice and the Manager must exercise sound independent judgment in adapting and applying leadership and clinical principles to specific situations. The Nurse Manager of Diabetes Program relies on experience, judgment and collaboration to plan and accomplish goals. The Nurse Manager of Diabetes Program ensures collaboration among physicians, administration, and members of the healthcare team to develop and evaluate programs and processes that adhere to the Standards of Medical Care in Diabetes. A critical component of this role is to generate and evaluate evidence for practice. The Nurse Manager of Diabetes Program assists in the advancement of the professional practice environment by communicating the NGHS nursing strategic direction and focusing on activities that support the nursing strategic direction. Provides nursing involvement support for VOICE and the NDNQI quality teams including coaching team members in their professional participation. Assists with overall clinical governance and other projects as assigned. Is a professional member of VOICE providing feedback on issues being addressed at meetings.

Minimum Qualifications:

  • Licensed to practice as an RN in Georgia. Required: Current Professional Certification as CDE or BC-ADM.
  • Required: A degree in Nursing (either BSN or MSN). Preferred: A Master's Degree (Nursing or other if possessing a BSN) within five years of hire.
  • Required: Professional organization membership within one month of hire into the role in the leadership or diabetes focus.
  • Must be a Registered Nurse with a minimum of 5 years of clinical experience. Two years should be progressively more responsible experience in management or clinical supervisory leadership role (managing other RN's). Academically or experientially prepared in areas of chronic disease care, patient education, and/or program management.
