PLS Nurse Job Summary/Objective:

A Personalized Local Support Nurse is responsible for managing the member's clinical experience from the point of request for a local referral to the desired outcome. To guide the members and utilize clinical expertise to guide them to adequate resources and care.

Job duties & responsibilities:

• Activate new member accounts, draft member medical history, and identify the goal of the local recommendation.

○ Offer additional services to the members when appropriate. Identify opportunities to guide the members with clinical expertise.

• Accurately enter the necessary information into Case Management Software.

• Facilitate local recommendations as needed and communicate with the members as needed for follow-up.

• Participate in weekly Care Team meetings or bi-weekly team meetings (when applicable). • Initial member outreach and guide the members with clinical expertise to the appropriate local provider within 2 hours from when the initial request was assigned out (business days). Using all methods of communication for daily follow-ups.

○ < 3 TAT

• Maintain and update the dashboard on an hourly daily basis.

• Purecloud coverage is 6.50 hours daily and a monthly average of 6.50 hours.

○ PC Answered calls 75% of the time.

○ PC Alert No Answer 25%

• 8 -5 shift

○ Rotating holidays/on-call

• Pilots and member reach outs.

Skills and Qualifications:

LVN with a minimum of 2-3 years and any experience in multimodality specialty/clinic setting or case management experience is a plus

Communication Skills, Time Management, Organization, Attention to Detail, Professionalism, Critical Thinking, Interpersonal Skills

Excellent communication and public speaking skills

Availability to adapt to the constant evolution
