RESPONSIBILITIES:

• Provide healthcare and wellness coordination
  

• Function as liaison and advocate for individuals' healthcare, wellbeing, and inclusion
  

• Develop long-term relationships with individuals served and their support systems
  

• Oversee the health status and physical conditions of individuals
  

• Provide staff training to meet the healthcare needs of the individuals served
  

• Manage transitions of care
  

• Work with pharmacy to ensure medication needs are met
  

• Ability to work independently and with a team
  

• May be required to float between locations

QUALIFICATIONS:

• Current Indiana license
  
• Valid Driver's License
  
• Experience in long-term or community nursing, preferred
