Are you passionate about community-based service while providing high quality care to all individuals? Then come join our Integration teams as a Lead Care Coordinator at Behavioral Health Network!

Lead Care Coordinators provide telephonic and in-person Care Coordination based on sound theories and principals. This includes all appropriate members in the individual's Integrated Care Team, including primary care providers, therapists, and others as indicated by the person served.

Lead Care Coordinators monitor the effectiveness of the Integrated Care Plans and coordinate with care team to make any changes as needed.

Behavioral Health Network (BHN) is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. We are dedicated to offering high quality, affordable and culturally appropriate care to people of all ages and income levels in our constituent communities.

Lead Care Coordinators at Behavioral Health Network have the following qualities:

• 6 months of work experience in a related behavioral health, human services and/or medical field (required)
• Knowledge of substance abuse, mental illness, and the interface between mental health and medical conditions.
• Must have Familiarity with Internet, email systems, and electronic health records (EHRs).
• Bilingual, Spanish and English Speaking (preferred)
• Must have leadership skills and ability to effectively relay information in writing, telephonically, and with verbal interactions,
