Compensation: $15.00 hourly

POSITION PURPOSE

This is a full-time, primarily mobile and community-based position in Children and Family Services involving a significant amount of direct service activities.

ESSENTIAL FUNCTIONS – JOB SPECIFIC

• Meets with children, youth and families at a frequency specified by treatment team to engage the child, youth and family in activities such as social and recreational enhancement in a variety of settings as related to a child’s treatment plan goals.
• Using a motivational, stage-matched approach, provide individualized services to youth and/or parents/caregivers based upon strengths, needs, abilities and preferences identified collaboratively with client/family, focused on supporting client’s movement toward recovery/resiliency goals
• Support client/family in completion of goals identified on Wellness Plans, reviewing the WP with client/family as required, reassessing client/family need, service choices available, community functioning and as needed for updating of goals and objectives as items are achieved, or as client/family goals change
• Provide educational information and skill building to client/family regarding areas of concern such as daily living skills, education, symptom management, communication/relationships with focus on improving functioning
• Communicates with child’s Integrated Health Specialist, the treatment team and supervisor(s) by attending scheduled meetings and supervision on a regular basis to discuss activities, changes in wellness plan goals, and significant observations related to the child or youth
• Documents all direct and indirect contact with all children, youth and families seen per Compass Health policy and procedures. Accurately maintains record of all expenditures, mileage, and time schedules and turns them in to the CPRC Youth Supervisor in a timely fashion, adhering to strict deadlines
• Consistently meet 100% Customer Care requirement
• Will seek culturally competent services and service providers to meet the unique cultural needs of each agency consumer and family member
• Carry out all other duties as assigned by the CPRC Youth Supervisor.
• Attend all training as required by Compass Health, Accreditors and Funders
• Regular attendance (except as excused)

EDUCATION

• High School required
• Bachelor’s degree in a social service-related field as recognized by the US Department of Education preferred

WORK EXPERIENCE

• At least two (2) years of related experience required if a bachelor’s degree is not obtained
• A valid driver's license and agency established minimum automobile coverage required
