Benchmark Human Services is expanding our support coordination (case management) operations into Utah! Since 1960, Benchmark has provided services to individuals with intellectual and developmental disabilities and through the years added employment services, behavioral health, home health, support coordination, crisis programs, along with consulting services. We are seeking qualified candidates for the Director of Support Coordination. This high profile position will lead the business development, staffing, training, support coordination services, compliance, and finances of the operation.

Job Responsibilities

  • Follow all policies and procedures set forth by the company and Utah Department of Human Services, Division of Services for People with Disabilities and Home and Community Based Services (HCBS) rules
  • Provide support coordination duties when staffing levels or consumer needs are not being met.
  • Hires, trains, manages, develops and coaches staff to maximize performance and outcomes
  • Establishes staffing levels and monitors caseloads to maximize performance and financial resources
  • Establishes and maintains positive relationships with individual, families, guardians, state officials and team members
  • Establish and monitor quality outcomes for consumers and the department
  • Ensures that all case records, plans and other related documents are properly maintained
  • Coordinate meetings with individual, families and guardians, and members of the interdisciplinary team as required
  • Records work and billing time in accordance with company policies
  • Tracks and monitors specified data points
  • Take necessary action to maximize revenue and limit expenses
  • Participate in short and long term planning process to ensure continued quality and financial success
  • Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.

Knowledge, Skills, Abilities
  • Fundamental understanding of the state's support coordination system
  • Fundamental understanding of the state's waiver programs
  • Must be able to work collaboratively while meeting individual performance goals
  • Must be able to verbally and non - verbally communicate with a broad audience of stakeholders
  • Ability and skill to use technology tools in the daily performance of duties
  • Effectively lead a team of case managers toward meeting customer service, compliance and financial outcomes
  • Analytical and critical thinking skills
  • Ability to manage financial resources
  • Understanding of HCBS rules, USTEPS, and the state's Waiver and Competitive and Integrated Employment programs

Qualifications
  • Bachelor degree in human services, social work, sociology, psychology
  • Hold a valid driver's license
  • Maintain auto insurance
  • Minimum of 2 years working in a support coordination role
  • Minimum of 3 years in a supervisory or management role
  • Preferred credentials include Accredited Case Manager (ACM) or Certified Case Manager (CCM)
  • Travel up to 75%
  • Successfully complete the state's annual training
