Job Description:

JOB SUMMARY

The Community Health Center, Inc. cares for 150,000 patients every year at sites across the state. We know that many of CHCI patients have complex lives and taking care of their own health or the health of family members can't always be the number one priority. CHCI is excited to have Community Health Workers (CHWs) join the team to help our patients tackle the everyday challenges they experience. Working to resolve common social determinants of health can improve our patients' overall wellness and their lives. These CHWs will be focused on working closely with Population Health and the clinical teams to improve colon cancer screening rates and education for patients. The CHWs will support patients in gaining a better understanding of the importance of colon cancer screening and support the patients in completing the screening of their choice. The CHWs will also help the patient navigate the health system for any follow-up care that is required.

ROLE AND RESPONSIBILITIES

  • Attend a CHW training program
  • Review referrals to determine eligibility for CHW support services.
  • Connect with patients directly at our health center sites, in the community, or in patient's homes to conduct a social determinants of health needs assessment.
  • Address challenges with social determinants of health that are impacting their ability to participate in preventative health care
  • Collaborate closely with the patient's healthcare team (including appropriate community resources), to assure that the patient is adhering to referrals and recommendations developed around their specific health needs
  • Develop metrics to properly evaluate goal progress.
  • Provide recommendations to the health care teams, based on level of care needed to optimize outcomes and minimize risk.
  • Complete follow-up calls and visits to patients as needed, on a case by case basis.
  • Assist patients with organizing their records, making follow-up appointments, and filling prescriptions.
  • Refer patients to Access to Care for support in applying for insurance
  • Help patients fill out applications for benefits to obtain additional assistance, such as SNAP.
  • Attend weekly case review meetings with supervisor and other CHWs.
  • Document each patient encounter in the patient's electronic health record.
  • Communicate regularly with patient's primary care team.
  • Attend community events and health fairs to educate about the importance of colon cancer screening
  • Attend CHC site specific meetings, as needed.
  • Other duties as reasonably assigned.

QUALIFICATIONS
  • Long- term resident of community served by CHC, with strong knowledge of the resources in that community.
  • Associates Degree required.
  • Bilingual- English and Spanish (preferred)
  • Excellent oral and written communication skills.
  • Ability to carry out written and oral instructions.
  • Demonstrated experience in the area of obtaining community resources/advocating on behalf of the patient.
  • Comfortable with computer data entry, experience working with an electronic health record preferred.
  • Able and willing to work part-time (30 hours/week)
  • Two references
  • Prior experience as an outreach worker preferred

  • Reliable transportation
  • Comfortable walking/driving in the community served
  • Working between an office and community setting
