1. Preparation and dispensing: Demonstrates competence in the performance and responsibilities related to providing the safe and appropriate dispensing and preparation of medications in accordance with federal, state, and local laws and in compliance with the requirements of the state's Board of Pharmacy.
o Prepares and labels medications according to applicable laws, regulations and policies.
o Checks work prepared by supportive personnel in a timely and accurate manner.
o Provides direction to supportive personnel for the preparation of complex drug preparations.
o Demonstrates a through knowledge of rules, regulations, policies, and laws pertaining to the preparation, storage, procurement, use and disposition of medications.
o Accurately documents all transactions involving controlled substances dispensing, preparation, and disposition according to established polices and procedures: reporting discrepancies, receiving obsolete inventory, delivering controlled substances and chain of custody documentation, recording transactions in the perpetual inventory system.
o Records recommendations, interventions, or other appropriate activities in Quantifi or other applicable intervention program.
o Compounds and accurately dispenses medications and sterile IV products including chemotherapeutic agents and TPN if applicable.
o Accurately monitors drug therapy regimens for contraindications, drug/drug interactions, food/drug interactions, allergies and appropriateness of drug dose and frequency.
o Supervises and directs pharmacy support personnel.
o Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.
o Identifies complications resulting from drug therapy and recommended treatment alternatives to minimize or negate the patient's adverse response.
o Reconciles patient's home medications for high risk medications upon patient admission and completes Medication Reconciliation within 24 hours.
o Participates inpatient discharge planning as needed/requested.
o Completes daily clinical monitoring reports in a timely manner.
o Completes Adverse Drug Reaction entries in Midas for applicable unit.
o If holds certification in BALS and ACLS, will respond to codes while on duty.
2. Provides education and information on pharmaceutical care and drug therapy.
. Provides accurate, adequate, and timely drug information to the professional staff of Wentworth Douglass Hospital.
o Provides a clinical inservice annually to pharmacy staff.
o Provides at least one educational inservice annually to nursing staff and/or other allied health professionals, Medical staff.
o Participates in or provides ongoing educational opportunities for pharmacy technicians.
3. Leadership and Professional Development
o Actively participates in a hospital committee.
o Is a member of local, state, national or international pharmacy organization.
o Productively works with Pharmacy Management as well as leadership of other disciplines in the development of department policies and procedures.
4. Functions as a Preceptor for pharmacy students and resident having met the criteria for Levels I-III Preceptor (score N/A if not applicable)
o Complete at least two CE credit per year in Preceptor Development.
o Complete one CE credit per year in professional development activity related to teaching, learning or mentoring.

Qualifications
Experience Minimum Required
• 3 to 5 years clinical practice
Experience Preferred/Desired
• 8 to 10 years health system experience
Education Minimum Required
• Graduate of an ACPE Accredited School of Pharmacy
• BS in Pharmacy
Education Preferred/Desired
• Pharm.D.
Training Minimum Required
• ASHP Approved Residency
Training Preferred/Desired
• ASHP Approved Residency or Equivalent Major healthcare experience
Special Skills Minimum Required
• Proficient in English, Excellent verbal and written communication skills
• Licensure and/or Certifications Required
• Pharmacist
