St. Joseph's Home Health Care extends the arm of skilled services from hospitals, clinics, and physician offices into the home. Working under the direction of your physician, our services are available 24 hours a day, 7 days a week.

St. Joseph's Home Health Care services can help to improve the quality of life for our patients and offer convenience for their families and physicians. Home health care can help patients to live as fully as possible at home with their loved ones.

Our family-centered approach includes teaching patients and their families about their care, coordinating additional services, and researching community resources.

Certified Home care aide

Basic Job Function Summary: Provides paraprofessional services to the organization's home care clients as developed by the primary nurse and as prescribed by their physician and in compliance with the state's Nurse Practice Act, and any other regulations defining home health aide / hospice aide services.

Minimum qualifications,
  • Must have current driver's license and reliable transportation to and from work site
  • Ability to work a weekend rotation every 3rd week and 3 holidays yearly
  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
  • Certified as HHA in state of New York preferred but not required(will pay for certification course & training)
  • Documentation of one year's employment as an aide in an institution or home care setting.
  • Meets Saint Joseph Home Care hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency's client and their family.
  • Must accept responsibility for maintaining skills, learning and adhering to agency's policies and procedures on an on-going basis.
