Job Title: RN - Case Management

Duties & Responsibilities: A Home Health Case Management RN oversees the care of patients receiving home health services, coordinating and managing their care plans to promote optimal health outcomes and independence. They work collaboratively with patients, families, caregivers, and interdisciplinary healthcare teams to provide comprehensive, patient-centered care in the home setting.

Minimum Requirements:

• Certified Home Health Care experience
• EPIC or Netsmart Required
• RN License (NY license ONLY)
• M-F 8a-430p
• Weekends per need
