Well-established Non- Profit organization whose primary focus is geared towards single adults, families and seniors who are indigent and/or homeless or at risk of losing their homes is looking for a Bilingual Case Managers !

Qualifications:

• Bachelors degree in Social Work, Psychology, Sociology, or related field-1+ years of direct social service experience
• Associate degree in Human Service, or related field with 3+ years of direct social service experience
• Knowledgeable of the plight of the homeless and special needs populations
• Knowledgeable of case work/case management methodologies
• Bilingual in English and Spanish

Responsibilities:

• Collaborate with program participants to identify a service plan that correlates with resolving presenting problems and obstacles to maintaining housing stability
• Answer city-wide phone hotline calls and provide real-time application assistance
• Assist in application preparation and submission to the program, service planning, advocacy and referral services
• Maintain weekly or bi-weekly contact with clients to follow-up on application status
