Join our team of competent and compassionate professionals as an Access Navigator in the Outpatient Department at Behavioral Health Network!

The Access Navigator has a unique role in community-based client care. In the CMHC model, you will build working relationships and provide in-the-moment interventions and referrals to support individuals. As an Access Navigator, you'll provide evidence-based care in the context of an integrated behavioral health team led by a Master's-Level. Access Navigators serve as advocates, providing needs assessment, case management, direct telehealth and/or in-person support, and help to access to needed services. The focus of the CMHC program is on quality of care rather than billable units of service. This position requires understanding of community resources and providers, both internal and external to BHN.

Who are we?

The CMHC program is a 2-year SAMHSA grant-funded pilot designed to reconceptualize outpatient community mental health treatment. Teams of service providers work collaboratively in order to expand access to coordinated and comprehensive wraparound services to clients via outcome-driven, evidence-based psychotherapy and social-emotional skills training and support. This program is designed to address the unmet behavioral and physical health needs of unserved and underserved adults and families, with a focus on serving vulnerable populations, and will. Services are delivered in a variety of settings and modalities, including clinics, homes, schools, outreach community locations, or via telehealth platforms.

Successful implementation of our CMHC grant award is poised to inform and transform future program operations. CMHC-funded positions will end at the conclusion of the grant funding in August 2023; however, sustainability planning is embedded in the program, and grant-funded positions will be prioritized for similar open positions within BHN.

Job Requirements:

What are we looking for in an Access Navigator?
  • Bilingual/bicultural candidates encouraged to apply!
  • High School diploma with 1-2 years of work experience in related health care field (required).
  • Associate's Degree with 1-2 years of work experience in related behavioral health field (preferred).
  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management (required).
  • Professional demeanor and phone skills in communicating with clients and care team clients (required).
  • Familiarity with electronic medical record (EMR,) internet, word processing and email systems (preferred).

**NOTICE**

In accordance with the BHN Mandatory COVID vaccine policy - all staff, interns, temps, residents and contractors are required to be fully vaccinated no later than day one of employment. Medical or religious exemptions will be offered and must be approved prior to starting work.

The same guidelines noted above apply during flu season (defined as December 1st - through end of flu season as announced by the CDC) for BHN's Mandatory Influenza Vaccine policy.

Additionally, BHN is a Tobacco Free workplace. All Staff, interns, temps, residents and contractors agree not to smell of or use Tobacco or ENDS products while on paid work time or within sight of any BHN facility.
